CareMessage Introduces Automated Gaps-in-Care Journeys for Safety-Net Organizations

SAN FRANCISCO, CA, September 4, 2024 — CareMessage, the largest patient activation platform in the United States for underserved populations, today announced the launch of their Automated Gaps-in-Care Journeys, an expansion to their market-leading innovative Health Equity Engine™. This new innovation integrates directly with EHRs to remove the burden of manual outreach to close gaps-in-care for safety-net organizations while increasing staff efficiency and improving clinical outcomes for patients.

The initial release will target patients with uncontrolled diabetes, addressing a critical UDS reporting measure for FQHCs and the seventh leading cause of death in the US. FQHC patients in particular are disproportionately affected by diabetes: 14% of the FQHC population is diagnosed with diabetes, compared to 11.6% of the general US population and nearly 30% of FQHC patients have uncontrolled diabetes.

CareMessage’s Automated Gaps-in-Care Journeys connect directly to the EHR used by the FQHC, FCC, or Tribal Health Center to automatically identify patients with uncontrolled diabetes and enroll them into messaging programs that bring patients in for lab visits, to attend a diabetes management program, or to educate them about diabetes management. Patients can receive messages through CareMessage’s built-in templates that utilize best practices developed from work with 400+ safety-net organizations in 45 states and 60+ languages. Alternatively, outreach can be customized to support the unique clinical and patient population outreach goals of the organization. This new feature quickly and easily re-engages patients on an ongoing basis to empower them to control their diabetes and enable the clinic to meet quality metrics without increasing the workload of already overburdened staff.
