Relevant Healthcare and CareMessage Announce Integration Partnership to Improve Health Outcomes

Today, Relevant Healthcare and CareMessage announced a partnership to advance their shared goal of helping safety-net organizations to improve health outcomes for their patients.
Relevant provides software and expert services to help community health centers improve patient care. The Relevant population health, reporting, and data platform is used by over 75 community health centers around the country to improve care for more than 1.2 million patients.
CareMessage is the largest patient activation platform for underserved populations in the United States. The robust CareMessage platform enables organizations to use mobile messaging to fill gaps in care, provide education, remind patients of upcoming appointments, automate follow up for open referrals, and provide one-to-one communication.
Via this new offering, customers of Relevant and CareMessage will be able to use CareMessage text messaging workflows as an integrated part of the Relevant software. Health centers will be able to close gaps in care, address social determinants of health, and gather patient satisfaction data with simple and automated communication.
